A Clinical Review of the Congenital Hand Anomalies

Abstract
66 patients of the congenital hand anomalies were admitted to Dept. of Plastic Surgery, Severance Hospital during the last 10 years from Jan 1966 to Dec 1975. These patients were clinically analyzed and the results, as follows.

1. The incidence of congenital hand anomalies was 0. 07% of total number of delivery.2. The familial incidence was about 7.6% of total admitted cases. 3. Male was predominated over female by the ratio of 1.9:1.
4. The incidence of each anomalies belong to the congenital hand anomalies was as
follows polydactyly in 44 cases (66.7%), syndactyly in 16 cases (24.3%), lobster
claw hand in 3 cases (4.5%); congenital flexion contracture of all fingers in I case (1. 5%), ring constriction band in 1 cases (1. 5%) and floating thumb in I case (1.5%).
5. Radial side extra thumb was most frequent form of polydactyly consisting of 43 cases, and 27 cases in the right side, 11 cases in the left side and 5 cases bilaterally. Most frequent type of polydactyly was type IV of about 45.8%.
6. In the syndactyly, most often affected site was ring and middle fingers. And r cases in the left side, 5 cases in the right side and 4 cases bilaterally.

7. Lobster claw hand was predominated over right side than left sicae and more unilateral than bilateral by the ratio of ¡®2: 1.

Associated congenital anomalies took place in 18 cases (27%), which was consisted of congenital anomalies of lower extremities in 6 cases (6. 0%), congenital head anomalies in 6 cases (7.5%), congenital heart disease in 3 cases (4.5%), anomalies of gastrointestinal tract in 2 cases (3. 0%), multiple congenital malformations in 2 cases (3.0).
9. Average of 49 operative cases was 6.3 years old. 10. 49 cases anesthesia was under the general with endotracheal anesthesia in 46 cases, brachial block in 2 cases, local anesthesia in 1 case.
11. The mean post-operative hospitalization date was 6.2 days.
12. 4 cases (8.2%) were readmitted for re-operation because of recurrence.
13. Post-operative complications should in cases (8.2%), which were consisted of flexion deformities due to scar contracture on the split thickness skin graft site, bony protrusion of metacarpal bone of the thumb and ulnar deviation of proximal pharyngeal bone of the thumb and webbing scar contracture.
